Dear Prospective Volunteer:

Thank you for your interest in volunteering with Samuel U. Rodgers Health Center. The support we receive from volunteers enables us accomplish our mission of providing high quality, compassionate and affordable healthcare for all. Attached you will find the Samuel U. Rodgers Health Center volunteer application packet. Volunteers are required to complete an application, provide 3 references (2 personal & 1 professional), complete a background screening and be available for an interview. Please find the following information in your application packet. If anything is missing, please contact my office for replacement.

  • Application Form
  • Release of Authorization Form
  • Volunteer Opportunities List
  • Reference form with envelope

Please check and return the following pieces of completed information and return to my office at your convenience:

  • Application Form
  • Release of Authorization Form
  • Volunteer Opportunities List
  • Copies of licenses or certifications
  • Copy of drivers license
  • Social Security number

Please do not submit your application until all components have been fully completed.
